We use Charm PHR as our secure platform for managing your Patient Health Record (PHR). Charm PHR is HIPAA-compliant and maintains strong privacy and security standards to protect your health information.

The platform offers a simple, user-friendly way to manage your information online. You can securely upload lab results, complete forms, schedule appointments, and communicate with your doctor through your CharmHealth account. After each visit, your doctor can also share visit summaries and handouts with you through the portal for easy access.

With Charm PHR, you can:

  • Add and manage health records like allergies, clinical problems, and other medical histories.

  • Message your doctor directly with medication or treatment-related questions

  • Securely share updates and documents with your doctor

  • Track symptoms and monitor your treatment progress over time

Patient Portal Access

Click here to access your patient protal. New patients will receive a welcome email with an invitation to sign in to your account. You can log in using the email address or mobile number you provided. For added security, you will be asked to enter your password and verify your identity.
